Prenatal exposure to a hepatitis A virus vaccine does not impact neurodevelopmental outcome in newborns. The recommended immunization schedules for infants against hepatitis A virus (HAV) contain the risk of in utero exposure. Because this exposure can lead to gestational or fetal-neonatal alloimmune thrombocytopenia (FNAIT) with delayed neurodevelopmental outcome, the purpose of this study was to assess the impact of in utero HAV infection on the neurodevelopment of preterm newborns. We performed a prospective study of all newborns of mothers vaccinated against HAV (n = 36) compared with matched newborns of nonvaccinated mothers (n = 36). We found no differences in gestational age, birthweight, and Apgar score between the 2 groups. The vaccination did not reduce the rate of neonatal thrombocytopenia. Neurodevelopmental outcome in preterm and term infants (n = 108) was assessed at 6 and 12 months of age using the Bayley Scales of Infant Development, Third Edition. At the age of 6 months, there was no significant difference in mean scores between the vaccinated and control groups. At 12 months, mean scores were significantly lower in the vaccinated than in the control group: the mean Mental Development Index (MDI) of 95.0 ± 10.0 vs. 112.0 ± 6.7 (P = 0.001); the mean Psychomotor Development Index (PDI) of 86.4 ± 12.0 vs. 100.7 ± 7.5 (P = 0.001); and the mean Motor Development Index (MDI) of 93.5 ± 11.7 vs. 113.5 ± 8.8 (P = 0.001). In conclusion, vaccination of pregnant women against HAV does not alter gestational age or birthweight. The rate of thrombocytopenia is not increased and does not interfere with the development of infants. However, the vaccination resulted in an impairment of neurodevelopmental outcome at the age of 12 months. Our data indicate the risk of delayed neurodevelopmental outcome in exposed infants.
